Re: Pre-Certification Service

This memo is to provide you with the understanding and contact information regarding this service for you and your patients. We are happy to identify insurance eligibility and perform the pre-certification process for the cardiac testing we perform at Cardiology Associates. Once the procedure is scheduled the Insurance Coordinator will start the pre-certification process. Should the insurance company require additional patient documentation, your office will be notified. At that time you may fax said documentation to the Insurance Coordinator by using fax number 908-429-8503.

The Insurance Coordinator is available to answer any questions and assist you in this process by calling number 908-722-6410 x311.

As a reminder for your patients, please remember to prepare the referral that is required for the testing visit in our office.
